Service Plan

The MHS Service Plan describes the services provided by MHS, and how to access them. It describes MHS programs, clients served, service goals, hours of operation, and location of services. The Service Plan was developed utilizing input from persons who have participated in MHS services, their immediate family members, and other healthcare and social-service organizations in the community.

CARF Accreditation

The Commission on Accreditation of Rehabilitation Facilities (CARF) awarded to Mental Health Services for Homeless Persons, Inc. (MHS) a three-year Accreditation Certificate, valid August 2006 through August 2009, for the following programs:

- Case Management/Services Coordination: Mental Health (Adults)
- Case Management/Services Coordination: Menalt Health (Children and Adolescents)
- Crisis Intervention: Mental Health (Adults)
- Crisis Intervention: Mental Health (Children and Adolescents)
- Outpatient Treatment: Mental Health (Adults)
- Outpatient Treatment: Mental Health (Children and Adolescents).

The Certificate of Accreditation was signed by Richard Forkosh, Chair, Board of Directors, and Brian J. Boon, Ph.D., President/CEO.

Internal Revenue Service 501(c)(3) Determination Letter

Click here to see the letter of 13 August 1997 from C. Ashley Bullar, District Director of the Internal Revenue Service. This letter states that an original determination letter was "issued in June 1989, granting your organization exemption from federal income tax under section 501(c)(3) of the Internal Revenue Code. That letter is still in effect."

State of Ohio Certificate of Good Standing

Click here to see the Certificate of Good Standing issued 7 March 2007 by Jennifer Brunner, Ohio Secretary of State. The Certificate states that MHS is an Ohio not-for-profit corporation in Cleveland, Ohio, incorporated 30 January 1989, and is in good standing. This indicates that MHS has filed required financial reports with the State of Ohio's Office of the Attorney General and the U.S. Internal Revenue Service for each of the past three years, and has paid all required fees for the past three years.
